Neither the secretary nor the manager have permissions or delegations set for
each other, but the notifications continue.

Can you tell me what I should request the server folks do to correct...

I was able to contact the manager in his new location, and had him check his
delegates. He currently has only his new secretary as a delegate.

Is there a way to remove his Calendar from the File...

The mailbox still exists - the manager moved to a different physical location.
